Terms and Conditions

The undersigned hereby irrevocably authorize Alan B. Tuttle, his assigns, licensees, heirs, and legal representatives, consent to the unrestricted use in perpetuity, for purposes including but not limited to copyright, publish, and use in all forms and media and in all manners for advertising, trade, or any other lawful purpose, images of the following property,  which I own and have full and sole authority to license for such uses, including but not limited to: Stories written, images of artwork created, or video captured based on doors in the Domiciles Project, regardless of whether said use is composite or distorted in character or form, whether said use is made in conjunction with my own name or with a fictitious name, or whether said use is made in publications for marketing, promotion or sale.

I waive any right that I may have to inspect or approve the finished version(s), including written copy that may be used in connection therewith.

I am of full age and have every right to contract in my own name with respect to the foregoing matters. I have read the above authorization and release prior to its execution and I am fully cognizant of its contents. 

This part of the authorization form shall serve as parental permission for the use of name, likeness, and/or photographic image of child/youth or the use of such child which the child owns and have full and sole authority to license for such uses, including but not limted to: Stories written, images of artwork created, or video captured based on doors in the Domiciles Project, regardless of whether said use is composite or distorted in character or form, whether said use is made in conjunction with my own name or with a fictitious name, or whether said use is made in publications for marketing, promotion or sale.

I grant permission to Alan B. Tuttle, his assigns, licensees, heirs, and legal representatives, to use my child’s likeness, photographic images and or any of the above-mentioned child’s work.
